Choices:
Register Account -
Email Confirmation Required Register Account -
Email Auto Confirm, Approval Required Register Account -
Email Auto Confirm, Auto Approve
If you chose any of the choices above and enable or not enable one, they all change to the same setting. Our site is notifying clients via email but is not telling them they have to confirm. The results in the client trying to keep entering the site without confirming.

Yes there are 3 types of emails. When you make enable or disable in one option, they all change to the same thing - it is true and correct as this setting relates to the same action - a client registers at you website.
Although yes we have 3 types of templates depending on the system configuration and that define the next actions of the client:
Register Account - Email Confirmation Required
Register Account - Email Auto Confirm, Approval Required
Register Account - Email Auto Confirm, Auto Approve
You can configure which type of template is used when you configure the system in this setting:
Configuration >> System Settings >> User Login & Registration
