4.0.16 Checkout Bug

When a customer gets to the "Confirm Appointment Request" page, the current date and time are shown instead of the date and time of the appointment they selected. It works fine if an admin/provider creates an appointment for a customer.

Could you also display the service on that screen too? The provider checkout displays the information much more clearly and offers the ability to change the service, date/time or provider without using the back button on the web browser. Would it be possible for the customer checkout procedure to look similar or at least more like v3.x?
