Can custom fields be added to the customer database? My client just needs a text field to keep notes on each customer. But the ability to have several custom fields could be useful for other clients.

yes sure, you can add any custom fields. Please have a look at this option in the demo:
1. Go to http://www.hitappoint.com/demo/
2. Choose "Admin"
3. Go to "Users >> Customize Form"
In this part you can add, delete, move up and down, and configure validation options for the fields in the customer information form.
Regarding the customer notes, you can create a new field named "notes" and make it hidden from the customer so that only providers and admin will see this info.

How do custom fields work with WordPress integration? I found this about extending WordPress user data: yoast.com/user-contact-fields-wp29. Would there be any way to use this with hitAppoint?

if integrated with WordPress, hitAppoint gets username, password, and full name from the WordPress database. All other info that is defined by hitAppoint custom fieds, is stored in the hitAppoint own database.
